I understand that I am not to leave my child at the ACES site unless a staff member is present.
I understand that my child will not be allowed to leave with an unauthorized person.
I agree to pay $5 per 15-minute increment (or any part of 15 minutes) for each occurrence that I pick up my child after class ends (12:30 PM morning / 5:00 PM afternoon). Example: 5–15 minutes late = $5 per occurrence.
I authorize medical treatment in emergencies if I cannot be reached.
I acknowledge ACES does not provide medical insurance and I assume all risks.
I permit my child’s photos/videos to be used for ACES publicity.
I understand my child may be removed for non-payment or disruptive behavior.
